WHAT DO FOUNTAINHEAD CAPITAL MANAGEMENT LLC, FOUNTAINHEAD AM LLC, AND FOUNTAINHEAD RETIREMENT SERVICES LLC DO WITH YOUR PERSONAL INFORMATION?

Why?

When you are no longer our customer, we continue to share your information as described in this notice.

All financial companies need to share clients’ personal information to run their everyday business. In the section below, we list the reasons financial companies can share their clients’ personal information; the reasons Fountainhead Capital Management, Fountainhead AM, and Fountainhead Retirement Services (collectively, “Fountainhead”) choose to share; and whether you can limit this sharing.

How does Fountainhead protect my personal information? To protect your personal information from unauthorized access and use, we use security measures that comply with federal law. These measures include computer safeguards and secured files and buildings.  We also restrict access to your personal information to those individuals who need to know the information to provide services and/or products to you.
How does Fountainhead collect my personal information? We collect your personal information, for example, when you
  • Open an account or transfer assets to us
  • Receive financial planning services
  • Provide account statements or other documents to use
Why can’t I limit all sharing? Federal law gives you the right to limit only
  • sharing for affiliates’ everyday business purposes—information about your creditworthiness
  • affiliates from using your information to market to you
  • sharing for nonaffiliates to market to you
State laws and individual companies may give you additional rights to limit sharing.  See below for more on your rights under state law.
Definitions
Affiliates Companies related by common ownership or control. They can be financial and nonfinancial companies.
  • Fountainhead Capital Management LLC, Fountainhead AM LLC, and Fountainhead Retirement Services LLC are related by common ownership
Nonaffiliates Companies not related by common ownership or control. They can be financial and nonfinancial companies.
  • Our nonaffiliated business partners include our custodian, TD Ameritrade
Joint marketing A formal agreement between nonaffiliated financial companies that together market financial products or services to you.
  • We do not have any such agreements in place.

Other important information

You may have privacy protections under applicable state laws, including those for California residents.  To the extent the state laws apply, we will comply with them when we share information about you, and in some case may be limited by you.   California Residents: The California Information Privacy Act provides additional protections to control whether we share some of your personal information.  In accordance with California law, we will not share information we collect about California residents with outside companies, except as permitted by law, such as with the consent of the customer or to service a customer’s accounts.
For Insurance Customers The term “Information” in this part means customer information obtained in an insurance transaction. We may give your information to state insurance officials, law enforcement, group policy holdings about claims experience or auditors as the law allows or requires. We may give your information to insurance support companies that may keep it or give it to others. We may share medical information so we can learn if you qualify for coverage, process claims or prevent fraud or if you say we can. To see your information, email [email protected].
